Audit and compliance are often discussed as if they are separate from care delivery, yet in adult social care they should function as practical tools for understanding whether support is safe, person-centred and consistent. Many providers can produce audit schedules, completed forms and action trackers, but still struggle to explain how these systems improve what happens on the floor. Providers working through audit and compliance in social care alongside wider thinking on quality standards and assurance frameworks will recognise that a credible framework must go beyond tick-box checking. It must show how leaders know what good looks like, how they test for it and how they respond when standards slip.

A strong audit and compliance framework therefore does three things at once. It creates consistent monitoring across key risk areas, gives managers a defensible basis for governance and turns findings into measurable improvement. Without all three, audit becomes paperwork and compliance becomes performative rather than protective.

What a defensible audit and compliance framework should include

In adult social care, audit systems should be proportionate, structured and closely linked to operational risk. High-value frameworks usually cover medicines, safeguarding, care planning, incidents, complaints, staffing, training, consent, restrictive practice and environmental safety. However, the strength of the framework lies not in the number of audits alone, but in how those audits connect to observation, supervision, action planning and governance review.

A useful framework should answer practical questions. What is being checked and why? How often is it reviewed? Who owns the process? What happens when concerns are identified? How do leaders know whether corrective action has worked? If the framework cannot answer these questions clearly, it is unlikely to provide strong assurance to commissioners or inspectors.

Operational example 1: structuring a medication audit cycle in domiciliary care

A domiciliary care provider supporting older adults and people with complex health needs found that medication audits were being completed monthly, but the results were too narrow to provide meaningful assurance. Managers could see whether MAR charts were signed, yet the process did not explain why errors occurred, whether staff understood escalation routes or whether service pressure was affecting safe administration.

The provider redesigned its medication audit framework so that file review was only one part of the process. Monthly MAR audits were combined with live spot checks, competency reassessment, incident trend analysis and review of care-plan instructions following hospital discharge or prescription change. The aim was to move from checking signatures to understanding practice.

Day-to-day detail mattered. Supervisors checked whether staff recorded administration at the point of care, whether refusals were coded accurately, whether time-sensitive medicines were flagged clearly and whether unfamiliar cover staff had enough package-specific information before visits. Audit findings were then grouped by worker, round, type of concern and level of risk.

Effectiveness was evidenced through improved MAR completion, fewer repeated errors on evening rounds and more targeted action planning. The provider could show that the framework identified causes, not just symptoms, and that governance oversight was grounded in operational reality.

Operational example 2: auditing safeguarding systems in supported living

A supported living service for adults with learning disabilities wanted stronger assurance that safeguarding procedures were being applied early enough to prevent escalation. Formal referrals were being made appropriately, but managers were less confident that low-level concerns, boundary issues and patterns of peer conflict were being captured consistently.

The provider introduced a safeguarding audit cycle that linked concern logs, incident reports, support-plan updates, supervision notes and restrictive practice review. Rather than limiting audit to whether forms had been completed, managers tested whether staff had recognised indicators of risk, recorded them proportionately and updated support strategies in response.

Operational review included looking at financial safeguarding issues, visitor patterns, changes in presentation after community access and whether restrictions introduced after incidents remained necessary and time-limited. Team leaders checked whether staff understood when positive risk-taking remained appropriate and when the threshold for safeguarding escalation had been crossed.

A residential care home supporting older adults recognised that its audit programme was strong on paperwork but weaker on lived experience. Documentation was generally compliant, yet family feedback suggested that some morning routines felt rushed and less person-centred during busy periods. Managers wanted an audit method that could evidence quality more directly.

The home introduced a dignity and practice audit combining observation, resident feedback, relative comments, care-note review and supervision themes. Instead of asking only whether care plans mentioned preferences, the audit tested whether those preferences were visible during support. Senior staff observed personal care, meal support and response to call bells with a focus on language, pace, privacy and choice.

Day-to-day monitoring looked at whether staff knocked before entering, explained support clearly, checked consent throughout, protected privacy and avoided task-led language when under pressure. How governance makes audit and compliance meaningful

An audit framework only becomes useful when findings are visible within governance. Leaders should be reviewing recurring themes, overdue actions, repeat risks and areas where audits conflict with feedback or incident data. Governance meetings should not simply note that audits were completed. They should test what the results mean, which risks matter most and whether action has led to sustained improvement.

This is also where compliance becomes more than rule-following. If audit repeatedly identifies weak handovers, poor consent recording or inconsistent escalation, governance should examine whether the issue lies in workforce confidence, procedure clarity, leadership oversight or service pressure. Re-audit and follow-up observation are then essential to verify that change has held.

Commissioner expectation

Commissioners expect audit and compliance systems to provide credible assurance that contracted services are safe, reliable and improving. They are likely to look for evidence that providers audit key risk areas systematically, analyse themes rather than isolated incidents and convert findings into measurable action. A provider that can explain how audit links to contract delivery, safeguarding oversight, workforce management and outcomes will usually be more persuasive than one relying on generic assurances.

Regulator / Inspector expectation

The Care Quality Commission expects providers to have effective systems and processes to assess, monitor and improve service quality. Inspectors may review audit tools and records, but they will also test whether leaders understand what findings mean and whether people’s lived experience matches the provider’s own assessment. Audit systems that produce activity without learning provide weak assurance. Audit systems that identify risk, prompt improvement and withstand triangulation support a stronger well-led judgement.

Turning audit into quality improvement

In adult social care, audit and compliance should function as a living quality system rather than a filing exercise. When providers build frameworks that connect checking, learning and improvement, they create stronger governance and better care. That is what makes audit defensible: not the volume of forms completed, but the clarity with which the system shows what is happening and what changes as a result.
