When ABI services experience breakdown, providers are often quick to complete investigations but slower to extract meaningful learning. Root cause analysis (RCA) is frequently treated as a compliance exercise rather than a tool for understanding why systems failed. Used properly, RCA helps providers strengthen ABI service models and care pathways and embed learning from service breakdown, recovery and improvement planning into everyday delivery.

Many investigations stop at the immediate trigger for failure: a specific incident, missed task or staff decision.
