Escalation triggers: KPIs must force decisions, not just reporting

KPIs without escalation triggers become passive reporting. Strong contracts define what happens when thresholds are breached. Examples include:

  • Backlog breaches trigger senior clinical review and interim contact controls.
  • Safeguarding action delays trigger immediate leadership escalation and mitigation plans.
  • Audit failure triggers targeted improvement work and re-audit within a defined timeframe.
