Community services sit at the sharp end of system pressure: more referrals, higher acuity, faster discharge expectations, and fewer staff. The result is often invisible harm—missed visits, reactive prioritisation, and caseloads that drift beyond safe limits. Demand management is not about refusing referrals. It is about ensuring the right work lands in the right place, with the right standard, and that the service can evidence why a pathway decision was made. Common demand controls in community services include:

  • Single point of access (SPA) with clear criteria and rapid triage.
  • Clinical validation for high-risk referrals (to prevent inappropriate “dumping” into community teams).
  • Time-limited intervention design with discharge rules and step-down planning from day one.
  • Redirection pathways for social/non-clinical needs (e.g., voluntary sector, social prescribing, local authority routes).

Where teams get stuck is failing to operationalise these controls. Criteria exist on paper, but staff are not supported to apply them consistently, or they are overridden informally due to system pressure.

They also expect providers to show how performance is monitored and improved over time, not just explained away by “pressure.”

Regulator / Inspector expectation (CQC): Inspectors expect to see that people receive safe, timely care and that delays, missed contacts, and deterioration risks are identified and managed. They look for effective governance: how risks are recorded, how leaders know when the service is unsafe, and how the provider learns and improves. Governance and assurance: what good looks like

Effective capacity governance is simple, visible, and repeatable. The strongest community services usually have:

  • Thresholds (caseload, response times, backlog age) agreed and understood.
  • Exception reporting so leaders see breaches early, not after complaints.
  • Clinical oversight of triage and discharge decisions, with sampling audits.
  • Workforce assurance (supervision, competency, and fatigue management).
  • System escalation that is documented, time-limited, and reviewed for learning.

Most importantly, governance must connect to day-to-day delivery. Staff need to know what changes when the service enters Amber or Red, and leaders need to be able to evidence why.
