Commissioners and inspectors rarely accept “we have improved” without evidence. After service failure, the provider’s credibility depends on the strength of its assurance system: what it measures, how it audits, and how it proves that changes are embedded in day-to-day practice. This evidence discipline is central to service failure, recovery and remedial action and must align with the delivery realities of different supported living service models.

This article sets out how to build an audit and assurance approach that demonstrates recovery is real, sustained and improving outcomes, not just compliance.

Why “assurance” matters more after failure

After failure, commissioners assume risk is higher until proven otherwise. They will look for a provider’s ability to detect issues early, intervene fast and learn consistently. Assurance is the mechanism that shows the service has moved from reactive crisis response to stable quality management.

Effective assurance answers three questions:

  • Are people safe and their rights protected?
  • Is practice consistent across staff, shifts and locations?
  • Are outcomes improving and sustained over time?

Designing an audit programme that supports recovery

Recovery audit programmes should be more frequent at the start and then taper as stability grows. Audits should not be limited to paperwork. They must test real delivery: staff understanding, practice observation and evidence of learning.

A practical recovery audit programme usually includes:

  • medicines management audit (including observed rounds)
  • safeguarding and incident response audit (thresholds, recording, learning)
  • restrictive practice review audit (MCA rationale, proportionality, review cadence)

Recovery must be governed through a clear rhythm. Providers should set a predictable cycle such as:

  • weekly: operational recovery meeting and audit review
  • monthly: quality and safeguarding panel with escalation decisions
  • quarterly: senior governance review and commissioner-facing assurance summary

Decision-making must be recorded. Commissioners and inspectors expect to see not just what the provider did, but how it decided what to prioritise and how it assured itself that actions worked.

Safeguarding, restrictive practices and rights-based evidence

During recovery, safeguarding and restrictive practice evidence must be rights-aware. Providers should be able to demonstrate that restrictions are individually justified, least restrictive and time-limited, with clear review points. Evidence must include the “why” not just the “what”.

Commissioner expectation

Commissioners expect evidence of recovery to be measurable, routine and sustained. They typically want assurance that improvements apply across all relevant placements, that risks remain controlled, and that governance is strong enough to prevent repeat failure.

Regulator / Inspector expectation

Inspectors (including CQC) expect providers to demonstrate an effective quality assurance system. This includes learning from incidents, consistent staff competence, safe medicines practice, and evidence that people experience improved outcomes and protected rights over time.

Making assurance usable, not performative

The strongest assurance systems are simple enough to be used daily, but robust enough to withstand scrutiny. Providers should avoid creating evidence solely for commissioners; instead, the evidence should naturally arise from good operational practice — consistent audits, strong supervision, and disciplined governance that uses what the data is saying.
